Parental Consent Waiver

  1. Confirm consent is required. If your child will be participating in shooting activities under the supervision of an adult other than yourself, this form must be completed before the visit.
  2. Gather the required information. You’ll need:
    • Parent/legal guardian’s full name
    • Parent’s phone number and email address
    • A photo or upload of the parent’s driver’s license
    • Child’s full name
    • Supervising adult’s name
    • Date of the scheduled activity
  3. Complete the form. Fill in each field accurately, confirming your relationship as the child’s parent/legal guardian.
  4. Sign and date. Provide your signature and the date to authorize the consent.
  5. Submit the form. Once submitted, the signed authorization is kept on file with WeShoot.
  6. Bring confirmation (if applicable). Have your confirmation or a copy of the completed form ready when you arrive, in case it’s requested at check-in.

Parental Consent Authorization

I hereby give my permission for (Child Name) to participate in shooting activities at WeShoot under the supervision of (Supervising Adult) on (Date of Activity), and I acknowledge that I fully understand the risks involved with the activity. I expressly understand that the aforementioned Activity includes the discharge of firearms and the firing of live ammunition.
